Evidence - Chimney-Technik bei juxtarenalem Aortenaneurysma (Ch-EVAR) - vascular surgery

Die sog. Chimney-Technik zur endovaskulären Versorgung von Aortenaneurysmen mit Beteiligung renoviszeraler Gefäße wurde 2003 als „Bailout“-Prozedur erstbeschrieben und 2007 als alternative endovaskuläres Therapieverfahren weiterentwickelt [1, 2]. Über die ersten Serien mit mehr als fünf abdominellen Fällen wurden von Ohrlander et al. in 2008, von Hiramoto et al. in 2009 und von Donas et al. in 2009 berichtet [3, 4, 5]. Die Publikationen belegen die Machbarkeit der Prozedur mit guten Ergebnissen bezüglich der 30-Tage-Morbidität und -Mortalität.

In 2013 berichteten zwei Zentren über die Ergebnisse eines CT-Angiographie-Follow-ups nach 24 Monaten bei 124 Hoch-Risiko-Patienten, bei denen die pararenalen Aneurysmen in 90 % der Fälle erfolgreich in der Chimney-Technik ausgeschaltet worden waren [6]. In einer weiteren 2-Zentren-Studie wurde belegt, dass auch die Kombination von zwei verschiedenen Materialien (Excluder (Gore) Stent-Graft mit Viabahn (Gore) und der Endurant-Prothese (Medtronic) mit dem Advanta V12 (Getinge)) eine gute Offenheitsrate von 95 % und eine geringe Inzidenz von Endoleaks Typ Ia (2 %) hat [7]. Zu dem gleichen Ergebnis kam auch eine In-vitro-Studie aus 2012 [8].

Zahlreiche Publikationen zu diesem Thema gingen aus dem PERICLES-Register hervor („Collected World Experience About the Performance of the Snorkel/Chimney Endovascular Technique in the Treatment of Complex Aortic Pathologies“). So wurde in 2015 die weltweit größte Erfahrung über diese Technik anhand von 517 Fällen publiziert. Die Offenheitsrate der Chimney-Grafts lag bei 94,1 % und die Regression des Durchmessers der versorgten Aneurysmen betrug durchschnittlich 6,5 cm. Zur Minimierung des Endoleak-Risikos die Gruppe das Schaffen einer neuen Landungszone von 20 mm [9]. Inzwischen sind zahlreiche weitere Publikationen aus dem Register hervorgegangen [10-19].

Das Auftreten von Endoleak Typ Ia wur­de von einigen Autoren als die „Achilles­ferse“ der Chimney-Behandlung angese­hen [20]. Unterschieden werden „high flow“ (HF) und „low flow“ (LF) Endoleaks. Beim HF tritt das Kontrastmittel außerhalb des aortalen Grafts unmittelbar nach der Applikation aus, bei LF erst deutlich verzögert in der späten Phase nach Kontrastmittelapplikation. Studien belegen, dass die LF-Endoleaks nach kurzer Zeit nicht mehr vorhanden sind und somit keine klinische Relevanz besitzen [21 - 24]. Zu einem ähnlichen Ergebnis kommt eine Studie aus Zürich (konservativ mit engmaschiger CT-Kontrolle) [25]. Bei HF-Endoleaks besteht hingegen eine absolute Indikation zur Intervention.
