Complications - Umbilical hernia, primary suture repair

  1. Intraoperative complications

    • Intraoperative bleeding
    • Bowel injury
      In case of accidental iatrogenic bowel injury during the procedure the lesion should be immediately closed by suture!
  2. Postoperative complications

    Overlooked bowel lesion
    Clinical signs: Patient does not recover from surgery, suffers from abdominal pain, nausea, guarding, and displays symptoms of peritonitis.
    Treatment: Revision surgery identifying the bowel lesion and closure by suture, possibly bowel resection. Peritonitis treatment
    Postoperative bowel paralysis
    Treatment: Administer prokinetic medication such as metoclopramide, neostigmine.
    Manifest ileus
    Clinical signs: Abdominal guarding and tenderness; free intraperitoneal gas on abdominal film; possibly elevated lactate on lab panel.
    Treatment: Revision surgery, identifying and managing the cause.
    Chronic pain
    Treatment: NSAIDs
    Wound infection
    Treatment: Initially nonsurgical with antibiotics; if not controlled with antibiotics, and in case of failure or no response to nonsurgical treatment: Reoperation
    Hematoma
    Treatment: Ultrasound monitoring; if growing in size possibly centesis or revision.
    Recurrence
    Treatment: If growing in size possibly revision surgery with mesh implant.
    Necrosis of umbilical skin (< 0.1%)
