Complications - Removal of a tendon sheath hygroma/Extensor tendon synovectomy with retinaculum plasty

  1. Intraoperative Complications

    • Tendon lesion
    • Lesion of the branches of the posterior antebrachial cutaneous nerve
    • Incomplete removal of the synovium with risk of recurrence

    Management

    • Primary tendon suture or tendon reconstruction
    • Careful and as complete as possible removal of the altered synovial tissue
  2. Postoperative Complications

    Acute Complications

    • Bleeding/Hematoma
    • Infection/Wound Healing Disorder

    Management

    • In case of relevant hematoma, operative revision
    • In case of infection, immediate operative revision
    • In case of wound healing disorder, operative revision depending on the severity of the findings

    Late Complications

    • Two-stage Tendon Rupture
    • Recurrence
    • Protracted Scar Pain/Neuroma Pain
    • Restriction of Movement
    • CRPS (Sudeck's Disease)

    Management

    • Tendon Reconstruction
    • In case of corresponding symptomatic complaints, re-resection
    • In case of scar pain/neuroma pain/neuropathic pain, watchful waiting, possibly desensitization therapy
    • In case of CRPS, appropriate therapy of the disease (see specialist literature)
