- Intraoperative Bleeding (5.4 % most common complication)
Due to injury to the renal or adrenal vein -> Renal vein should preferably be oversewn or reconstructed if necessary conversion. The adrenal vein drains on the left into the renal vein. Its course is significantly longer than on the right side. The bleeding can usually be better controlled and the vein controlledly ligated.
- All organs that are mobilized for the correct exposure of the adrenal gland can be injured:
- Capsular lesions of the kidney -> hemostatic measures (coagulation, hemostasis with fibrin glue or sealing with fibrin-coated collagen fleece)
- Injury to the left colonic flexure -> preferably oversewing, in case of large complex defects of the intestinal wall or devascularization → segmental resection with anastomosis if necessary protective stoma.
- Injury to the pancreatic tail -> atraumatic suture, drainage
- Injury to the spleen: Avoidance through subtle preparation, hemostatic measures (coagulation, hemostyptics) Attempt at organ preservation, splenectomy as ultima ratio.
- Diaphragm injury with pleural lesion can lead to a pneumothorax or pneumomediastinum, in case of hemodynamic relevance insertion of a chest drain.
- Hormonal reaction, especially catecholamine-induced with intraoperative circulatory dysregulation in pheochromocytoma.
