Complications - Liver cyst deroofing, laparoscopic

  1. Intraoperative Complications

    Bleedings
    from small vessels: Electrocoagulation or Clipping
    from large liver vessels: if necessary, use of a laparoscopic ultrasound with vessel Doppler function, hemostasis with clips

    Bile leaks
    Management with clips or suture

    Injury to adjacent organs like stomach, colon, diaphragm or bile ducts can occur in very rare cases.

  2. Postoperative Complications

    • Postoperative bleeding or hematoma due to insufficient hemostasis or coagulation disorders.
    • Bile leakage or bilioma, if small bile ducts in the cyst wall were opened during the procedure: Increase in liver and infection parameters, increasing abdominal complaints. ERCP with stent placement, possibly also placement of a drainage under ultrasound or CT guidance.
    • Infections (wound infections or intra-abdominal abscesses): Antibiotic treatment, possibly drainage in case of persistent secretion.
    • Pleural effusion/ Pneumonia: Chest X-ray, ultrasound. First negative fluid balance, postoperative mobilization and breathing exercises, in case of respiratory insufficiency possibly thoracic drainage system.
    • Ascites formation and leakage through the trocar sites: Evaluate drainage insertion with fascial closure.
    • Recurrence of the liver cyst: Especially with incomplete deroofing or multiple cysts. In such cases, a renewed intervention may be necessary. Recurrences occur less frequently in laparoscopic procedures if the deroofing is performed generously.
    • Injury to adjacent organs such as stomach, colon, diaphragm or bile ducts can occur in rare cases.
